== Prognosis == The average progression rate from one stage of liver fibrosis to the next in those with MASH is estimated to be seven years. The course of progression varies with different clinical manifestations among individuals. Fibrosis in those with MASH progressed more rapidly than in those with MASLD. The risk of cirrhosis, liver cancer, liver-specific death, and overall death is higher in those with MASH as compared to MASLD. All-cause mortality in those with MASH is 25.5 deaths per 1000 person-years. Liver-specific mortality is 11.7 deaths per 1000 person-years. In one study that examined people over 15 years, 11% of those with MASH developed cirrhosis as compared to less than 1% of people with MASLD. Other estimates have shown that MASLD and MASH were found to worsen with cirrhosis in respectively 2–3% and 15–20% of the people over a 10–20 year period. Obesity predicts a worse long-term outcome than for lean individuals. In the Asia-Pacific region, about 25% of MASLD cases progress to MASH within three years, but only a low proportion (3.7%) develop advanced liver fibrosis. An international study showed that people with MASLD with advanced fibrosis had a 10-year survival rate of 81.5%. MASLD is a risk factor for liver fibrosis, hypertension, chronic kidney disease, atrial fibrillation, myocardial infarction, ischemic stroke, and death from cardiovascular causes based on very-low to low-quality evidence from observational studies. === Depression === The primary use of trazodone is the treatment of unipolar major depression with or without anxiety. Data from open and double-blind trials suggest that the antidepressant efficacy of trazodone is comparable to that of amitriptyline, doxepin, and mianserin. Furthermore, trazodone has shown anxiolytic properties, low cardiotoxicity, and relatively mild side effects. Because trazodone has minimal anticholinergic activity, it was especially welcomed as a treatment for geriatric patients with depression when it first became available. Three double-blind studies reported trazodone had antidepressant efficacy similar to that of other antidepressants in geriatric patients. Unfortunately, a side effect of trazodone, orthostatic hypotension, may cause dizziness and increase the risk of falling. This may have devastating consequences for elderly patients. Therefore, this side effect, along with sedation, often makes trazodone less acceptable for this population compared to newer compounds that share its lack of anticholinergic activity (but not the rest of its side effect profile). Still, trazodone is often helpful for geriatric patients with depression who have severe agitation and insomnia. Trazodone is usually used at a dosage of 150 to 300 mg/day for the treatment of depression. Lower doses have also been used to augment other antidepressants or when initiating therapy. Higher doses, up to 600 mg/day, have been used in more severe cases of depression (in hospitalized patients, for example).

=== Pharmacodynamics === Nor-LSD showed 5- to 29-fold lower affinity for the serotonin 5-HT2 receptor compared to LSD (Ki = 30–158 nM vs. 5.4 nM, respectively). It also showed affinity for the serotonin 5-HT1 receptor. In another more recent study however, nor-LSD showed similar or even higher affinities, activational potencies, and/or efficacies at the serotonin 5-HT1A, 5-HT2A, and 5-HT2B receptors as LSD, whereas it showed 36-fold lower affinity for the serotonin 5-HT2C receptor compared to LSD. Nor-LSD failed to completely substitute for LSD in rodent drug discrimination tests even at very high doses. The greatest degree of substitution with nor-LSD was 75% at a dose of 7,420 nM/kg, whereas 100% substitution occurred with LSD at a dose of 186 nM/kg (a 40-fold lower dose). The ED50Tooltip median effective dose was 2,594 nM/kg for nor-LSD and 46 nM/kg for LSD. Hence, nor-LSD was approximately 56-fold less potent than LSD in terms of producing LSD-like effects in rodents and failed to produce full LSD-like effects even at the highest assessed dose. == Limitations == Chemotherapy does not always work, and even when it is useful, it may not completely destroy the cancer. People frequently fail to understand its limitations. In one study of people who had been newly diagnosed with incurable, stage 4 cancer, more than two-thirds of people with lung cancer and more than four-fifths of people with colorectal cancer still believed that chemotherapy was likely to cure their cancer. The blood–brain barrier poses an obstacle to delivery of chemotherapy to the brain. This is because the brain has an extensive system in place to protect it from harmful chemicals. Drug transporters can pump out drugs from the brain and brain's blood vessel cells into the cerebrospinal fluid and blood circulation. These transporters pump out most chemotherapy drugs, which reduces their efficacy for treatment of brain tumors. Only small lipophilic alkylating agents such as lomustine or temozolomide are able to cross this blood–brain barrier. Blood vessels in tumors are very different from those seen in normal tissues. As a tumor grows, tumor cells furthest away from the blood vessels become low in oxygen (hypoxic). To counteract this they then signal for new blood vessels to grow. The newly formed tumor vasculature is poorly formed and does not deliver an adequate blood supply to all areas of the tumor. This leads to issues with drug delivery because many drugs will be delivered to the tumor by the circulatory system.
